A NYSMF Teacher Recommendation is required of all first-time applicants. You may use our standard Teacher Recommendation Form, or request that your recommender email your recommendation directly to us at info@nysmf.org.

If you are using the online form, click the SUBMIT button at the bottom of that page to send your application, and to pay the required $75 APPLICATION FEE. (via Paypal or major credit card. Please click here for
complete information.)
Any materials that need to be sent to complete your application (Financial Aid Form, Performance recordings, etc.) may be sent via any of the following methods. Please be certain to include your NAME and EMAIL ADDRESS on everything you send to NYSMF, so we can be sure to put it together properly when it gets to us!
EMAIL. You may scan and send any forms, recordings, etc. to us at application@nysmf.org.
FAX. You may fax any document to us toll free at (888) 632-3221. Our fax line is available 24 hours a day, 7 days a week.
