For your convenience, you may fill out the NYSMF Student Application Form ONLINE. Click here to jump to that page. You'll save time, hassle, postage, paper,
and reduce your carbon footprint to a mere toe-print in the
sand!
A NYSMF Teacher Recommendation is required of all first-time applicants. You
may use our standard Teacher Recommendation Form, or request that your recommender email your recommendation directly to us at
info@nysmf.org.

If you are using the online form, click the SUBMIT button at the bottom of that
page to send your application, and to pay the required $75
APPLICATION FEE. (via Paypal or major credit card. Please click here for complete information.)
Any materials that need to be sent to complete your application (Financial Aid
Form, Performance recordings, etc.) may be sent via any of
the following methods. Please be certain to include your NAME and EMAIL ADDRESS on everything you send
to NYSMF, so we can be sure to put it together properly when
it gets to us!
EMAIL. You may scan and send any forms, recordings, etc. to us at application@nysmf.org.
FAX. You may fax any document to us toll free at (888) 632-3221. Our fax line is
available 24 hours a day, 7 days a week.

- For Session 1: June 24, 2012
- For Session 2: July 7, 2012
- For Session 3: July 21, 2012
Late applications must be accompanied by a $100 late application fee and payment
in full in a cashiers' or bank check, or via credit card.
