print, and complete the NYSMF Application Forms. ALL
FORMS ARE REQUIRED FROM ALL APPLICANTS.
Includes
the following forms:
- Application
Form
- Teacher
Recommendation Form
To
be filled out by your teacher, conductor, lesson
teacher, etc. (May be sent separately)
- Permission
Form
Provides your parent or guardian's permission to participate
in various recreational activities, as well as listing
the basic NYSMF Code of Conduct.
- Financial
Aid/Scholarship Form
Required for consideration for NYSMF
Scholarships or need-based financial aid.
- SCHOLARSHIP
APPLICANTS MUST INCLUDE A DEMO TAPE THAT
INCLUDES 1-2 movements of concertos (or short pieces)
of contrasting styles
- Health
Form Checklist
- Health
Examination Form
- Medication

Vaccination Response Form
